REGISTRATION

*Universal Preschool*
We are a Universal Preschool participating program! If your child will be 4 years-old by 10/1/23, they are eligible for Universal Preschool. Eligibility is not dependent on income level and families can receive at least 15 free hours/week of preschool for the 2023-2024 school year. To apply, click HERE. You must apply separately with UPK in addition to us. This article has some helpful information on the application process and what to expect.
Registration Process: 
Registration for the 2023-2024 school year is currently open.

1. Please fill out and submit a Registration Form. Registration forms can be found below. Forms can be emailed to lsregistration@stlukeshr.com, mailed, or dropped off in person.

2. Once registration is open for the new school year, you will receive an email confirming a spot for your child. Please allow up to a few weeks to receive the email during the month of January. At this time a Non-Refundable School Supply Fee is due per child (UPK does not cover the fee). This fee covers supplies, books, and administration fees for the upcoming school year. Fees can be paid through our Smartcare app or by check. If you are a new family, a Smartcare account will be created for your family when your child's spot is confirmed.

3. Later in the spring you will be emailed a link to our Registration Packet that needs to be completed by June 15th. We will also collect medical forms and one month's tuition payment (this will be counted as May 2024 tuition) on August 1st. ​First month's tuition will be due September 1, 2023. We do not collect tuition for the month of August. If your child is eligible for UPK, this will be taken into account. More details to come.

Other Important Registration Information:
  • Classes are arranged according to birthday, gender, and age appropriateness.​
  • Signed medical paperwork must be received by the first day of school in order for your child to start. If your child has an allergy and needs medication to be kept at school, all paperwork and medication must be submitted by the first day of school in order for your child to start.
  • You will receive your teacher/class assignment in August.

This will be emailed to enrolled families in the late spring 2023.
2022-2023 Registration Form
Registration Packet – due by JUNE 15th ($25 late fee applied for late packets) 
You will receive an email by the end of May to complete the registration packet through a program called Hello Sign.

Medical Forms – due by AUGUST 1st ($25 late fee applied for late forms and children cannot start school until forms are received)
We require a General Health Appraisal form signed and dated by YOU and your child’s HEALTHCARE PROVIDER, as well as an Immunization Record.

*If you are a returning family and submitted updated health forms from your child’s most recent well check (within 1 year from their last birthday), you do not need to submit new forms. If your child has an August birthday, please submit forms from their well check in August 2021 and then submit updated forms after their well check in August 2022.
 
Medical forms can be:
  1. Faxed directly from your child’s healthcare provider to us at 303-470-5615
  2. Emailed to lsregistration(a)stlukeshr.com
  3. Dropped off in person to the Little School mailbox located near the north side door of the church
